to enrich our multi-professional team in the outpatient care of people with disabilities in the area of integration assistance according to SGB IX.

Horizonte SH GmbH & Co KG is a recognised provider that now offers various integration assistance services at eleven locations in the districts of Stormarn, Bad Segeberg, Pinneberg, Steinburg and the city of Neumünster. The aim of our work is to support people with a mental or psychological impairment to lead a self-determined life in their own living space and to promote inclusion.

We are looking for a permanent employee

in part-time or full-time (30-39 hours)

for the location Südliches Pinneberg/Raum Wedel/Elmshorn.

Your tasks:

You will accompany our clients in the context of compensatory assistance with the aim of implementing individual participation planning in everyday life (takeover and accompaniment), in particular:

- Assistance in coping with everyday life and especially in the household
- Help with orientation in the living environment
- Exploration and accompaniment of leisure activities
- Accompaniment in participation in cultural and sporting events
- Proper preparation of documentation and
performance records
- Participation in service meetings and close
cooperation with educators

What we expect:

A specific professional qualification is not necessary. You must have personal aptitude and ideally experience in working with people with disabilities. Driving licence class B is available.
